Tired all the time, and your bloodwork came back normal

It is a particular kind of frustrating. You are exhausted in a way that sleep does not fix, you go to your doctor, they run a panel, and everything comes back within range. The implication — sometimes said out loud — is that there is nothing to find.

There usually is something to find. But standard panels are built to answer a specific question, and it is not the question you are asking.

What a standard panel is for

Conventional reference ranges are designed primarily to catch disease. They are typically derived from a broad population and drawn wide enough that a result outside them is a strong signal that something is wrong. That is genuinely useful, and it is why these tests are ordered. But a result sitting just inside the edge of a wide range is reported the same way as one sitting comfortably in the middle: as normal.

Where fatigue tends to hide

  • Iron status — a ferritin at the very bottom of range behaves differently from one in the middle
  • Thyroid function beyond a single screening marker
  • Blood sugar handling across the day, not just one fasting number
  • B12 and folate status, and whether they are being used properly
  • The stress-response pattern, which is about rhythm rather than a single value
  • Sleep quality as distinct from sleep quantity

Patterns matter more than single values

What I find most useful is rarely one dramatic result. It is a set of values that are each individually unremarkable but that together describe a body under load — running on adrenaline, short on a couple of key raw materials, digesting poorly, and sleeping badly as a consequence. Any one of those in isolation gets waved through. Together they explain quite a lot.

Before you order anything

If you are tired, please do not skip the ordinary explanations. Sleep apnoea, anaemia, thyroid disease, depression, medication side effects and simply not sleeping enough are common, and they are properly managed through conventional medicine. Functional testing is most valuable once those have been considered, not instead of considering them.
